The 30-Minute Clock and the Magnesium Ladder

Acute-onset severe hypertension is a hypertensive emergency because of the risk of haemorrhagic stroke, and it should be treated within 30 to 60 minutes, ideally within 30. Pick the right first-line agent for the patient in front of you and run the escalating schedule against a clock. Then climb the magnesium toxicity ladder, run the hourly hold parameters, put the response to toxicity in order, and test a panel against the HELLP definition.

Treat the pressure, then watch the magnesium

Recheck the blood pressure at 10-minute intervals during treatment, and do not aim for normotension — a target of roughly 140–150 over 90–100 mmHg preserves uteroplacental perfusion.
