Per Kilogram, Then Capped

Children are not small adults, and paediatric dosing is not adult dosing scaled by eye. Every dose is per kilogram of actual measured weight — and then checked against the maximum adult dose, which becomes the ceiling. Work a dose and watch the cap engage, run maintenance fluids by the 4-2-1 rule, read a CCHD pulse-oximetry screen, match every prophylaxis to its deadline, and correct twelve pieces of discharge teaching.

Work the numbers

Acetaminophen is dosed at 10 to 15 mg/kg/dose orally every 4 to 6 hours, to a daily maximum of 75 mg/kg/day, and the maximum single adult dose is 1000 mg.
