Opioid Rotation, Symptom Console and Double Effect

There is no analgesic ceiling for a pure mu agonist — the right dose is the one that relieves the symptom without unacceptable toxicity. Rotate an opioid and watch what happens if you forget the incomplete cross-tolerance reduction, work the nine symptoms that define a death and name the monitoring parameter for each, then classify five cases against the doctrine of double effect and find the one that is the real ethical failure.
