Say-It Bench: Boundaries, Splitting and the Team

A person who learned that closeness is unpredictable is soothed by a relationship with visible edges. Boundaries are not an obstacle to the therapy — on a behavioural unit they largely are the therapy. Pick the sentence you would really say at six moments; all four options then open, each with the technique it uses or the trap it falls into named. Then build a three-part limit of your own and sort what you may hand to assistive personnel.

Delegation on one-to-one observation

Ms. R is on one-to-one observation for self-injury risk and you are working with an experienced assistive personnel colleague. For each task, decide whether it can be delegated. This is a licensure question and a safety question at the same time.

Validate then structure — in that order. A limit delivered before the feeling is acknowledged is heard as rejection and escalates. The same limit delivered after “I can see this is unbearable right now” is usually accepted. Validation is not agreement with the behaviour; it is accurate recognition of the emotion driving it.
When the team describes two different patients, the split is in the team. Splitting is the inability to hold good and bad qualities in one person at once — a defence formed when a child had to keep a dangerous caregiver good in their mind in order to survive. It is not a strategy being run on your ward. The repair is structural, never disciplinary: one written plan every discipline can quote, limits agreed in a team meeting rather than at the bedside, and one designated person who communicates changes.

Build a three-part limit

It is 1940. She has asked you to sit with her and you have a med pass at 2000. Assemble your sentence from the three parts — and watch what happens to it if you leave one out.
1 · Acknowledge the feeling
2 · State the limit concretely
3 · Offer what you can do
Choose one line from each part and your sentence assembles here.

Traps you chose

The drift test. Staying an extra twenty minutes. A personal number or a social media handle. Sharing your own life to prove you understand. A gift accepted or given. Becoming the one nurse who really gets this patient. Each step feels compassionate and each is a betrayal in waiting, because you cannot sustain it. The rule is one line long: if you would not do it for every patient on the unit and could not write it in the chart, do not do it.
